Sculptra, Radiesse and Bellafill.

What the collagen evidence supports, where the elastin claims outrun it, and what you give up when you choose a filler that cannot be dissolved.

Short version: collagen stimulators work differently from hyaluronic acid filler. They don't add volume so much as provoke your own tissue into making more of its own, gradually. The collagen evidence for that is decent. The elastin claims stapled to it in marketing are not. And none of these three can be dissolved, which is the trade you're actually making.

Locally, 43 providers list Sculptra and 29 list Radiesse, which puts biostimulators well ahead of most hyaluronic acid brands other than Juvederm. They're being sold hard right now. That's worth understanding before you sit down, because the sales conversation for these is genuinely different from the one for lip filler.

Materials and durations are the FDA's own descriptions. Two details from the same source that rarely make it into a consultation: poly-L-lactic acid is the same polymer used in absorbable stitches and bone screws, and calcium hydroxylapatite shows up on x-rays and can obscure underlying features. Tell your dentist and your radiologist.

What Sculptra actually costs locally

Per vial, published locally, Sculptra runs $675 to $800, and the median is $800. But per-vial pricing is close to meaningless here, because the protocol is a series. Most treatment plans use two to four vials across several sessions, and results arrive over weeks rather than on the drive home.

So a realistic Sculptra course locally is somewhere between $1,900 and $3,200, and a provider quoting you "$800" without saying how many vials your plan needs hasn't answered the question. Ask for the course price. If they won't estimate it, that's an answer too.

The collagen evidence, separated from the elastin marketing

Here's where I want to be careful, because this is exactly the kind of claim that gets repeated until it sounds settled.

Collagen: reasonably supported. Histology studies do show increased collagen density after treatment with both PLLA and CaHA, with fibres appearing thicker and better organised. That's a real, measurable tissue change, and it's the mechanism these products are approved on.

Elastin: much weaker. You'll see "stimulates collagen and elastin" on local provider pages and in manufacturer material. When you follow those claims back, the sources are mostly small studies with manufacturer involvement, clinic blog posts, and review articles written by people who sell the treatment. Rebuilding a genuine elastic fibre network in aged skin is a hard problem, and the evidence that these products do it is not in the same weight class as the collagen evidence.

You'll also see a specific number, "up to 66.5 percent more collagen," attached to Sculptra. That's a manufacturer figure. It may well be right. It is not an independent finding, and it deserves the same scepticism as any number a company publishes about its own product.

This is the part of my background that's actually useful here. Medical technologists spend their careers asking whether a result means what someone claims it means. "Studies show" is not a study, and a histology slide showing more collagen at three months is a different claim from "this rebuilds your skin."

The trade you're making: none of these dissolve

Hyaluronidase breaks down hyaluronic acid. It does nothing to PLLA, CaHA or PMMA. There is no equivalent enzyme for any of the three products on this page.

Practically, that means:

None of that makes biostimulators a bad choice. Their whole appeal is that they do something HA can't, gradually, and for the right face that's genuinely the better purchase. But a first-time filler patient being routed straight to Sculptra should know they've skipped past the reversible option, and should be told why.

Bellafill, and what "permanent" means

Exactly one provider within an hour of Boulder lists Bellafill. It's the only permanent filler material the FDA has approved, and its approval is narrower than most people expect: non-absorbable fillers are approved only for nasolabial folds and cheek acne scars. Not lips, not cheeks generally, not jawlines.

The carrier is bovine collagen, which matters if you have allergies. The FDA specifically tells patients to alert their provider before any filler if they're allergic to collagen, to eggs, or to animal products, and Bellafill is where that warning bites hardest. Ask about allergy testing before treatment, not after.

My honest read on it being listed by one provider out of 235: the local market has quietly decided that permanent is a hard sell in a field where tastes change every few years. Faces that were filled to the fashion of 2018 are a reasonable argument for reversibility.

What to ask before you book a biostimulator

  1. How many vials or syringes does my plan need, and what's the total course cost?
  2. Why this product rather than hyaluronic acid for my face?
  3. How many of these have you personally done, and what's your nodule rate?
  4. What happens if I get a nodule, and is that follow-up included?
  5. When will I actually see the result, and when do I stop seeing it?
  6. Which physician is delegating this treatment?
