Exosomes

Exosomes are small vesicles released by cells, and the products sold in aesthetics are derived from cultured cell lines rather than from you. They are typically applied topically immediately after microneedling, when the skin barrier is open, and sometimes injected.

This is the one category on this site where we think the regulatory position belongs in the first paragraph rather than a footnote. The FDA states that there are currently no FDA-approved exosome products, that exosomes used to treat conditions in humans are regulated as drugs and biological products requiring premarket review, and it has issued public safety notifications after reports of serious adverse events in patients given unapproved products marketed as containing exosomes. A published case series has also described persistent redness, nodules and scarring after intradermal injection of unapproved exosome formulations.

None of that means every provider offering them is careless, and it is not a reason to avoid a clinic. It does mean this is a treatment to ask direct questions about, and to discuss with a clinician who knows your history before agreeing to it.

What to ask a provider

• What product is this, who manufactures it, and what is its regulatory status?
• Is it being applied topically or injected?
• What adverse reactions have you seen, and what would you do about one?
